A variety of CGA forms are available for download. View or download a PDF copy of each form using the appropriate link below. Adobe Acrobat Reader™ is required for viewing. Acrobat Reader may be downloaded for free. The PDF form files will open in a new browser window.
Riders: You have 60 days following your first show to sign up for Year Ends Awards and have the rides count. The show year begins after the State Championship Show each year in early August, and ends with the next State Championship Show. You must have current CGA membership (or sign up that day) to ride at any CGA show. |

State Championship Show (State Sponsored show - Premium List and Entry Form)
July 24 - 31 - State Championship Show!! The State Championship Show is held at the San Benito County Fairgrounds at Bolado Park in Hollister. Entries close Friday, July 2nd. Entries close July 2nd, and there is NO late entry! However, you may add additional classes until 3:00 pm the day before the event, with an additional $5 late fee per add on. Download your premium list here and your entry form here. This is a CGA State sponsored show! Districts must contact Patrick Hoffman to schedule their event to be worked. Pay With Credit Card (Credit Card Authorization Form)
CGA now accepts Visa, Mastercard, Discover and American Express! How do you use your credit card? Download the Credit Card Authorization Form, complete the form, and submit the completed form to CGA with the applicable entry or sign-up form. There is a credit card processing fee of $10 for all charges, except $25 for State Show entries. Credit card charges cannot be accepted over the phone.

Membership Form
CGA membership must be renewed yearly based on the original date of membership. You can't ride if you're not a member! Anyone may apply for CGA membership.

Year End Awards Form (Level 1, Level 2 & Top Rating)
Sign up for a Level 1 award to earn a silver buckle or silver breast collar set. Sign up for a Level 2 award to earn a silver bracelet or silver bridle set. And... sign up for a Top Rating award and earn a CGA jacket!

Family Team Awards Form
Sign up more than one member of a family, and all can earn matching CGA jackets with the family name embroidered on the back!

Race of Champions Application Form
Run for the Hall of Fame! You will have the opportunity to earn a gorgeous Hall of Fame buckle with your name and your horse's name engraved on it! And/or, win a Championship (best average in an event) or Reserve Championship for an embroidered CGA jacket. And/or be an event Record Holder for a medallion with your winning event and time engraved on the back. There are these and more awards to win in this elite group, but you must be fast!

Senior Elite Application Form
Are you old enough to run for the Senior Elite Hall of Fame! You will have the opportunity to earn a gorgeous Senior Elite Hall of Fame buckle with your name and your horse's name engraved on it! Local District Annual Contract
The Annual Contract must be completed and signed by each district. This is the document that says, yes, your district will uphold CGA rules, and lists district officers, district delegate and contact persons.
